Replacement Certificates

Please complete the Request for Replacement Certificate by Approved Centre form and return to the OCN for the attention of the Security Industry Team at the address below with the accompanying payment.  Payment is required by a cheque made payable to 'National Open College Network' for £25.00 per learner.  If we have previously agreed to invoice you for the amount then please enclose a note informing us with the form.  Please note that we cannot invoice for less than £50.00.

Please ensure you write your cheque guarantee card number on the reverse of the cheque.
